Q: How do I transfer my medical file/records from another physician’s office?
A: Fill out the “Records Tranfer Request Form” for records from another physician’s office either on our website under “Patient Forms” tab or while in the office. The request will be forwarded to the other physician’s office within 3 business days.
Q: How can I pay my bill?
A: We accept cash, check or credit/debit cards. You can pay your bill online through Phreesia by clicking the green “Make a Payment” button on the top of this page.
Q: When making an appointment can I request to see a specific doctor?
A: For routine visits, the practice makes its best effort to have patients see the same provider. If it is an emergency, we will schedule the earliest available time.

Q: What are your fees if I do not have insurance?
A: The fee for an initial office visit for a new patient is usually between $100-$200 depending on the complexity of the visit. Additional fees may apply for any procedures performed during that doctor’s visit. We ask that patients bring $200 for the initial visit. For subsequent visits, we ask the patient to bring $125. At the end of the visit, if charges are less, patient will be refunded the difference. For self-pay patients, we accept cash or debit/credit card only. The only credit card the office DOES NOT accept is American Express.
Q: Are there any other practices affiliated with this location and doctors?
A: No, this is our only practice location.
Q: Are there weekend hours or services available?
A: We offer Saturday and Sunday office hours. A provider is always available for emergencies after hours.
